Referral Process
All referrals should be faxed to our office with the following information included:
Referring Physician - If an NP or PA, include the Directing Physician as well
Most recent office note
Any diagnostics relating to the referral
Any other pertinent information
Most recent office note
Any diagnostics relating to the referral
Any other pertinent information
Once we receive the referral, we will make every attempt to contact the patient within 24 business hours to
establish an appointment for their earliest convenience.
After the patient is seen, a consultation letter with plan is faxed back to the referring physician for their chart.
